Influenza/COVID Vaccinations

Maple Valley Pharmacy will provide Influenza and/or COVID vaccinations to students whose parents opt them in by filling out the Influenza Consent Form. Only students whose parent/guardian have opted them in will be allowed to receive these vaccinations. If you'd like your child to also receive the COVID vaccination, please make a note on the form requesting that as well. If you are unable to print the form, please stop by any of the school offices to request a printed copy.
